Service Completion Agreement

Purpose

This agreement ensures that providers certify honest, professional completion of work before invoicing is triggered.

By submitting a completion form, you confirm that:

   All services listed were completed to the best of your professional ability.

   Truthful Documentation

   All notes, photos, and details accurately represent the work performed.

   Unresolved Issues

   You have disclosed any known issues or follow-up items to the client before submitting.

   Invoicing & Disputes

   You understand that this submission triggers a client invoice and starts the two-day dispute window. Payout occurs only after client payment and any necessary Network review.

   Service Tier

   The service tier you confirm on this form may differ from what the client originally selected. You confirm the tier reflects the actual complexity and scope of work completed. The client retains the right to dispute a tier adjustment within the two-day window.
